To apply as a Basic Member, you must complete Form B – Basic Member Application.

If you are paying by credit card, you may send your application by e-mail. In this case, enter your credit card information, and e-mail your application as an attachment. If you are applying by mail, enter your credit card information or enclose a check or money order payable in US dollars, and send all  materials to:  THAI    P.O.
