Section 1: Personal Information
Enter the requested information in the white spaces. Use the tab key to jump
from one box to the next.
To be a member of THAI, you must have an e-mail address (or use a friend’s
e-mail address).

Section 2: Professional Information
Enter the name and location of each teacher or school you have studied with,
the dates of study, and the total study time for each location. Use the tab key
to skip from one column to the next.

Answer the other questions on page 2 by entering the information in each box.
Type an "X" in all boxes that apply.
If you teach Nuat Thai, enter the school name, location and province.
Read the declaration at the end of the form, and enter your name and the
date.

Once your application materials are received by THAI, you will be sent a
confirmation of receipt by e-mail. If for some reason, more information is
requested in order to process your application, a THAI representative will
contact you and request the additional information.
